GENDER IDENTITY AND SEXUAL IDENTITY IN WOMEN WITH ASD

In women with an autism spectrum disorder (ASD), the development of sexual and gender identity often differs from that of neurotypical women. It may be helpful to know for women with ASD with a non-heterosexual orientation or with gender dysphoria/variance, that this is more common and what the possible underlying mechanisms are. It is also important for care providers to know more about the sexual identity and gender identity of women with ASD, so they can adjust their approach accordingly. This article, therefore, aims to provide an overview of current knowledge about gender identity and sexual identity in women with ASD. This involves both an overview of the existing literature as well as experiences from clinical practice.

Bij vrouwen met een autismespectrumstoornis verloopt de ontwikkeling van de seksuele en genderidentiteit vaak anders dan bij andere vrouwen. Wat zijn de gevolgen hiervan en hoe kun je hiermee rekening houden in de spreekkamer? Annelies Spek en collega’s geven een overzicht van de beschikbare literatuur en gaan in op implicaties voor de klinische praktijk. ‘Uit wetenschappelijk onderzoek komen duidelijke aanwijzingen naar voren dat er bij vrouwen met ASS vaker sprake is van gendervariantie.’

Een autismespectrumstoornis (ASS) komt voor bij ongeveer 1% van de bevolking (Baird et al., 2006)5. Lange tijd is gedacht dat ASS ongeveer acht tot tien keer vaker voorkomt bij mannen dan bij vrouwen. Een recente systematische review wijst echter op een man-vrouwverhouding van ongeveer 3:1 (Loomes, Hull & Mandy, 2017)20. Dit betekent dat er naar verhouding meer vrouwen zijn met ASS dan eerder werd aangenomen.

Wetenschappelijk onderzoek en klinische ervaringen laten zien dat ASS zich bij vrouwen op een andere manier kan uiten dan bij mannen. Dit heeft waarschijnlijk bijgedragen aan de onderschatting van de prevalentie. Zo zien we bij vrouwen met ASS meer compensatiegedrag op sociaal gebied en lijken zij gevoeliger voor sensorische prikkels dan mannen met ASS (Lai et al., 2011; Spek & Goosen, 2013)1926. Ook lijkt er sprake van andere comorbide problematiek (Lai et al., 2011)19. Internaliserende problemen, zoals angst- en stemmingsklachten, blijken bijvoorbeeld vaker voor te komen bij meisjes en vrouwen met ASS,
