Vragenlijsten als hulpmiddel bij de diagnostiek van somatisch-symptoomstoornissen

Met de komst van de DSM-5 wordt het domein van de somatoforme stoornissen vervangen door de somatisch-symptoomstoornissen en aanverwante stoornissen. De somatischsymptoomstoornis wordt gekenmerkt door lichamelijke klachten die gepaard gaan met disfunctionele gedachten, gevoelens, en/of gedragingen. In deze bijdrage bieden Anne van Gilst en collega’s een beknopt overzicht van vragenlijsten die door clinici en onderzoekers gebruikt kunnen worden voor het in kaart brengen van deze maladaptieve reacties op lichamelijke klachten.

Met de komst van de DSM-5 wordt het domein van de somatoforme stoornissen vervangen door somatisch-symptoomstoornissen en aanverwante stoornissen (APA, 2013; Van der Feltz-Cornelis & Van Houdenhove, 2014)117. De belangrijkste diagnose in dit domein is de somatisch-symptoomstoornis. Deze vervangt de DSM somatisatiestoornis, pijnstoornis en ongedifferentieerde somatoforme stoornis. Kenmerk van de somatisch-symptoomstoornis is het langdurig aanwezig zijn van één of meer lichamelijke klachten die lijdensdruk veroorzaken en/of het dagelijks functioneren significant beïnvloeden. Daarnaast dient er sprake te zijn van overmatige gedachten, gevoelens, en gedragingen met betrekking tot de lichamelijke klachten.

Dit laatste punt markeert een belangrijk verschil met de DSM-5. Het is voor de diagnose niet relevant of de klachten al dan niet verklaard kunnen worden door een somatische aandoening; van belang is de maladaptieve reactie op de lichamelijke klachten.

Clinici en onderzoekers kunnen bij de diagnostiek van somatisch-symptoomstoornissen gebruikmaken van vragenlijsten die systematisch dit soort maladaptieve reacties in kaart brengen. Dit artikel heeft als doel een
